If you are looking for forms used by the Pasco Discovery Coalition, you've found the right page.

Coalition Member Agreement Form

One important aspect of our coalition is that of having pals and organizations make agreements to become members. To learn more about our coalition you can see information about our partners, the coalition leadership, and participating.

We would really like you to consider partnering with us as we work to reduce substance abuse. Becoming a member means joining attending at least 80% of our monthly coalition meetings. You can always look at our calendar for the upcoming coalition meetings.

We also require all of our members to participate in at least one subcommittee and one activity each year while they are a member. You can learn more about out subcommittees as well.

Training Request Form

One major benefit of being a member of the coalition, is the ability to participate in trainings. Sometimes these trainings are local, such as the SAPTS training and sometimes they are national trainings like CADCA. In 2014 we sent a group to Florida. This year, the CADCA Mid-Year training is in Las Vegas. You can read more about it at CADCA's Mid-Year Training Institute's website.
